Health Home Care Coordinator/Care Manager for Adults

Ohel is seeking a Care Coordinator to manage the care of adults enrolled in Ohel’s Health Home program. The Care Coordinator will assess the adult's physical, mental health and social services needs and will be responsible for developing an integrated plan of care, working collaboratively with medical, behavioral, educational and social service providers. The Care Coordinator will also provide care coordination and health promotion, transitional care and follow up, individual and family support, referrals to community and social support services as well as the use of health information technology to link services. This full time position is based in Brooklyn.

Salary:  Bachelors Level $50,000

Masters Level $55,000
